A study published in The BMJ has found a link between glucagon-like peptide-1 (GLP-1) receptor agonists and an increased risk of hair loss in adults with type 2 diabetes. These drugs, used to treat diabetes and obesity, were associated with a higher incidence of alopecia compared to other diabetes medications. The study specifically highlights concerns with commonly prescribed GLP-1 receptor agonists such as semaglutide and tirzepatide, which have previously been anecdotally linked to hair loss but lacked full comparative research.

Study Design and Findings

Researchers analyzed electronic patient data from the University of Pennsylvania Health System, comparing 12,004 patients on GLP-1 receptor agonists with 15,221 using SGLT-2 inhibitors and 11,964 GLP-1 users with 11,233 on DPP-4 inhibitors. The study period ran from January 2019 to September 2024. Notably, GLP-1 users were younger, with a mean age of 58, compared to 65 for SGLT-2 inhibitor users and 67 for DPP-4 inhibitor users. They also had a significantly higher body mass index (36.2) compared to 32.3 and 31.3 in the other groups, respectively. These demographic differences were accounted for in the analysis to ensure a fair comparison.

After adjusting for factors like age, sex, ethnicity, pre-existing conditions, other medication use, and body mass index, GLP-1 users had a 37% higher risk of alopecia compared to SGLT-2 inhibitor users and a 68% higher risk than DPP-4 inhibitor users. This translates to 6.91 cases per 1,000 person-years for GLP-1 users versus 5.04 for SGLT-2 inhibitors and 3.89 for DPP-4 inhibitors. Further analyses revealed that the increased risk was specifically for non-scarring alopecia, where hair follicles remain intact, allowing for potential regrowth. The risk of non-scarring alopecia was 53% higher for GLP-1 users compared to SGLT-2 inhibitor users and 72% higher compared to DPP-4 inhibitor users.

Potential Mechanisms and Limitations

The authors suggest that rapid weight loss, a common side effect of GLP-1 drugs, could trigger hair shedding. This may also lead to nutrient deficiencies like iron or zinc, which are essential for maintaining the hair growth cycle. Hormonal changes induced by GLP-1 receptor agonists might also contribute, though the exact mechanisms require further investigation. The study’s reliance on electronic health records limited its ability to capture detailed clinical information, such as the severity, extent, duration, and reversibility of hair loss after treatment cessation. Additionally, unmeasured factors, such as lifestyle changes or concurrent medications, could have influenced the results.

Despite these limitations, the study’s strengths include its large, representative cohort and the use of high-quality data from a major health system. The findings were consistent across multiple analyses, enhancing their reliability. The researchers emphasize that while the absolute risk of hair loss remains low, the study provides systematic evidence to support earlier anecdotal reports, filling a critical gap in diabetes and obesity treatment research.

The researchers conclude that their work extends earlier safety signals, offering a clearer picture of this adverse effect. This information can help clinicians and patients engage in shared decision-making, balancing therapeutic benefits with potential side effects. Future studies should focus on understanding the underlying mechanisms of GLP-1-induced alopecia and exploring strategies to mitigate this risk, ensuring safer and more effective treatment options for individuals with type 2 diabetes and obesity.
